Patient Follow-up

Outcomes of the prescription event

As outcomes of the consultation events, Dr Lisa creates the prescription event

  • Medical events/actions that Dr Lisa prescripted
    • Specialist consultation
    • Biology exam
    • radio
    • hospitalization
  • “Patient Self Follow-up Schedule” is directly issued from the prescription event
    • Drug intake schedule
    • Capturing data related to health state (fever, arterial pressure, glycemic check, heart beat pulses…)
    • Patient contribution: notes, remarks, photos

Upon action of each responsible, it creates various medical events

ChatCal synchronizes with authorized participants all these information that enrich the patient dossier in BlueSafe

Patient Follow-up

The self follow-up dossier

The “Self follow up Dossier” gathers the “Drug Intake schedule” as well as the various actions to perform as fever catch, weight input, glycemic daily measures, blood pressure, heart pulse beat, diet respect… All these information are either keyed-in or transmitted automatically from medical devices

If parents and care givers assist the patient the “Self Follow-up Dossier” is synchronized with their devices, they can report in ChatCal the performed actions and warn the prescribing doctor in case of emergency

The patient

  • is reminded of the actions
  • confirms drug intake
  • can keyin the self check-up data (fever, glycemic rate)
  • add notes and remarks as pain rating
  • describe symptom and state (voice recording)
  • other drug intake (analgesic…)
  • take photos of wound healing evolution

ChatCal synchronizes these data with the patient dossier in BlueSafe, and with the assistants (thus avoiding duplicated action or omission) and eventually send a warning to the prescribing doctor if values are out of range.

Dr Lisa can check this follow up information at any time. She has a global view of the patient dossier, eventually filtered per duisease, as

  • a list of event
  • a calendar
  • a Gant chart
  • and graph of medical measures overtime in correlation with treatments

Risky patient Follow-up

Prevention of accident

Similar to hospital monitoring, risky patient might be equipped with

  • Permanent devices (as pacemaker, health wristband, wearable medical control device…)
  • External device that the patient use to perform regular exam (as daily glycemic exam, arterial pressure…) and key-in the information

These devices will provide regular information to the patient smartphone using BlueTooth or NFC.

ChatCal mobile collects these information in the Self Follow-up dossier and synchronize with the patient dossier and the assistants devices

A specialized app in the smartphone can collect relevant data, send  collection anonymously this collection to a medical IA server that will  analyze deeply the data and eventually generates an urgent medical report to the prescribing doctor.

This follow up features can prevent from medical accident as diabetic coma, cerebral accident…

Risky patient smartphone can benefit of activation of the ICE feature (In Case of Emergency) customized to their particular case.

Sync-n-Clinic

Preventive actions

Sync-n-clinics is a variant of the walk-in-clinics and similar to the “self patient Follow-up” event. This event records medical information when non-healthy symptoms are noticed and synchronize them in real time or on regular basis with doctors or AI Diagnostice engine.

Sync-n-clinic aims to prevent worsening of the disease and to reduce health cost.

Mary watch over her child, who seems to be sick

Synch-n-Clinic event

She creates an event based on the “Sync-n-clinic” template. Fill it with her pediatrician or referal doctor. She captures the fever (or uses a medical IOT device) . The medical IOT device send regularly captured data to the mother’s smartphone (Bluetooth, NFC, WIFI…). From time to time or on demand, ChatCal Mobile on her smartphone send anonimously the collection of data to an AI medical analytics server.

AI diagnostic medical server

The Artificial Intelligence server analyze the data asking Mary to provide with complementary information. As outcomes the server provide on a regular basis:

  • advises to Mary,
    • to inform on her child health state
    • to perform actions (drug intake, fever catch…)
    • to take comfort in her anxiety
    • and depending on the case, to launch a tele-consultation, a walk-in clinic or a consultation request
  • In case of emergency the pre-diagnostic, is sent to the pediatrician

Patient Dossier

All update of an active event is recorded in the Patient Dossier At this stage the consultation event is closed, this event has been securely deleted from all devices of all participants and is kept securely available in the Patient Dossier in BlueSafe the Record management solution.

ICA

In Case of Accident the emergency doctor has the needed medical information to consult on site the victims and can contact the refering doctor
